VXI Repair Authorization Request Form

You may request a Repair Authorization Number online at any time. Simply follow the steps below.


Each out-of-warranty product will incur a $28.50 repair charge. You will be contacted for payment information if it is not already on file with VXI. Any products to be returned to VXI for a refund must be handled through VXI customer service at 800-742-8588.

*Our apologies, we do not repair out of warranty Voice It products. These recorders carry a 1 year manufacturer's warranty from the time of purchase. Additionally, we are unable to sell you a new recorder as we are no longer manufacturing new Voice It products. We appreciate your cooperation.

 

1. Please enter your customer code:
Your customer code is located on your packing list or invoices that you have received from VXI. If you are unable to locate your customer code, please contact VXI Customer Service at 800-742-8588 and we will be please to provide you with your code.

2. Single Product Submission. Please enter the product's serial number here:
The six digit serial number on a headset is found on the earpiece under the ear cushion. On an amplifier, it is found on the bottom of the unit. If your product doesn't have a serial number please use the Multiple Product Submission option below.
3. Please use the "Multiple Product Submission" button below if you are returning more than one item or if your product does not have a serial number.
